摘要:
目的探讨正常白蛋白尿期2型糖尿病患者血清miR|16|5p水平与成纤维细胞生长因子2(FGF2)的相关性及其可能意义。方法纳入78例正常白蛋白尿的2型糖尿病患者为病例组,另选78例健康体检者为对照组,所有对象随访24月后,检测剩余对象(病例组60例,对照组70例)入组时血清miR|16|5p与FGF2表达,并分析二者的相关性,再通过生物信息学数据库探讨二者之间的关系。同时观测所有对象随访前后尿标本白蛋白/肌酐比率(UACR)、肾脏超声肾脏最大横切面积、血管阻力指数的变化。根据测定血清miR|16|5p水平将病例组分为<0.665组(A组)和≥0.665组(B组),比较A,B组和对照组(C组)随访前后各指标的变化。结果(1)病例(A,B)组患者血清miR|16|5p水平均明显低于C组(P<0.01,P<0.05),而空腹血糖(FPG)和糖化血红蛋白(HbA1c)、UACR、肾血管阻力指数、肾最大横切面积和FGF2均高于C组(P<0.05,P<0.01);(2)病例(A+B)组患者血清miR|16|5p与FGF2水平呈明显负相关(r=-0.683,P<0.01);(3)生物信息学数据库提示,miR|16|5p可靶向结合FGF2基因3′|UTR区;(4)A组比B组出现更多尿白蛋白转化发生率(43.90% vs 15.79%,P<0.05)、肾脏最大横切面积增加更多[(5.21±4.60) vs (3.25±3.98),P<0.05]、肾血管阻力指数增加更明显(P均<0.05)。结论早期糖尿病肾病患者低表达的miR|16|5p与上调的FGF2密切相关,可能通过上调FGF2表达参与早期肾损伤。
Abstract:
ObjectiveTo investigate the correlation between serum levels of microRNA|16|5p (miR|16|5P) and fibroblast growth factor 2 (FGF2) in patients with type 2 diabetes mellitus in normal albuminuria and its possible significance.Methods78 patients with type 2 diabetes mellitus with normal albuminuria were enrolled as the case group and 78 healthy subjects as the control group.After 24 months’ follow|up, the serum levels of miR|16|5p and FGF2 in the remaining subjects (60 cases in the case group, 70 cases in the control group) were detected, and the correlation between them was analyzed.The relationship between them was discussed through bioinformatics database.At the same time, the changes in urinary albumin/creatinine ratio (UACR), maximum transverse area of kidney, and vascular resistance index were observed before and after follow|up.The patients were divided into <0.665 group (group A) and ≥0.665 group (group B) based on the levels of serum miR|16|5p.The changes of each index before and after follow|up were compared between group A, group B and control group (group C).Results(1) The serum levels of miR|16|5p in case group (A+B) were significantly lower than those in group C (P<0.01, P<0.05), while the levels of fasting plasma glucose (FPG), glycosylated hemoglobin (HbAlc), FGF2, UACR, renal vascular resistance index, and maximum transverse area of kidney in case group (A+B) were higher than those in group C (P<0.05, P<0.01).(2) The serum levels of miR|16|5p in case group (A+B) were negatively correlated with the levels of FGF2 (r=-0.683, P<0.01).(3) The bioinformatics database indicated that miR|16|5p could target the 3′|UTR region of the FGF2 gene.(4) The incidence of urinary albumin transformation was higher in group A than in group B (43.90% vs 15.79%, P<0.05), the maximum cross|sectional area of kidney increased more [(5.21±4.60) vs (3.25±3.98), P<0.05], and the renal vascular resistance index increased more significantly (all P<0.05).ConclusionThe low serum levels of miR|16|5p in patients with early diabetic nephropathy is closely related to the up|regulation of FGF2, which may be involved in early renal injury through up|regulation of FGF2.
