目的 研究关节镜下Lasso缝合钩联合FasT-Fix修复内侧半月板桶柄样撕裂的临床疗效。 方法 收集于关节镜下应用缝合钩技术联合FasT-Fix修复并行二次关节镜手术探查的内侧半月板桶柄样撕裂患者16例,其中合并前交叉韧带(ACL)损伤11例,予行关节镜下ACL重建术。术后综合临床症状、MRI检查及二次关节镜检查结果评估半月板修复治疗的临床效果。随访时间平均为18月。 结果 16例中,1例(6.25%)出现深蹲时膝关节疼痛,余15例无疼痛、积液、交锁、关节间隙压痛等表现。16例均于二次关节镜探查前行MRI检查,结果显示,完全愈合11例(68.75%),部分愈合4例(25%),不愈合1例(6.25%)。二次关节镜探查结果显示,完全愈合13例(81.25%),部分愈合2例(12.5%),不愈合1例(6.25%)。总成功率(完全愈合+部分愈合)为93.75%,失效率为6.25%。 结论 关节镜下Lasso缝合钩联合FasT-Fix治疗内侧半月板桶柄样撕裂疗效满意。
Abstract:
Objective To study the clinical efficacy of arthroscopic Lasso sutures combined with FasT-Fix in repairing the bucket-handle tear of medial meniscus. Methods From September 2014 to October 2018, 48 cases of arthroscopic Lasso sutures technique combined with FasT-Fix were used to repair the bucket-handle tear of medial meniscus. A total of 16 patients underwent arthroscopic second-look. The age of these patients ranged from 17 to 45 years with an average of 27 years. There were 9 male patients and 7 female patients. 11 patients with arthroscopic anterior cruciate ligament injury underwent arthroscopic anterior cruciate ligament reconstruction. The clinical effects of meniscal repair were evaluated by clinical evaluation, MRI assessment and arthroscopic second-look. Results A total of 16 patients were followed up for an average of 18 months. Of the 16 patients, only 1(6.25%)had knee pain in deep squats, and the remaining 15 patients had no pain, effusion, interlocking, joint space tenderness or other complications. 16 patients underwent MRI before arthroscopic second-look. The results of the MRI study: 11(68.75%)patients with complete heal, 4(25%)patients with partial heal, and only one(6.25%)with non-heal. According to the arthroscopic second-look, 13 patients(81.25%)were completely healed in the 16 cases of bucket-handle tears, 2(12.5%)were partially healed, and 1(6.25%)were non-healing. The total success rate(including complete healing and partial healing)was 93.75% and the failure rate was 6.25%. Conclusion Arthroscopic Lasso sutures combined with FasT-Fix is effective in the treatment for bucket-handle tear of medial meniscus.
