摘要:
目的探讨强直性脊柱炎(AS)患者血清中软骨寡聚基质蛋白(COMP)的水平及其与疾病活动性的相关性。方法收集确诊的AS患者60例(活动组和非活动组各30例)及健康者(对照组)30例。检测2组患者和对照组的血清COMP水平,其中检测12例活动期AS患者治疗24周前后血清COMP水平; 同时检测全部患者血清中红细胞沉降率(ESR)、C反应蛋白(CRP)等相关指标,并进行骶髂关节CT分级。分析3组血清COMP水平,并探讨与其他指标的相关性。结果活动组血清COMP水平高于非活动组和对照组,非活动组高于对照组( P<0.05)。相关性分析显示,血清COMP水平与Bath强直性脊柱炎疾病活动指数、脊柱痛VAS呈正相关( P<0.05),而与年龄、病程、血小板、球蛋白及CRP,ESR,CT等级无明显相关(P>0.05)。结论血清COMP可反映AS患者的炎症和疾病活动性,结合CRP及ESR可以更好地评价AS患者的炎症反应、疾病活动性并且在一定程度上指导治疗。
Abstract:
ObjectiveTo discuss the level of COMP in serum of AS patients and its correlation with disease activity.MethodsThis study collected 60 patients with ankylosing spondylitis(30 in each active and inactive group)and 30 in healthy control group.To detect the COMP concentration in serum of AS patients, healthy control group and the COMP concentration of 12 active AS patients(active group)before and after 24 weeks of treatment.At the same time, the levels of ESR, CRP and other laboratory indicators in patients’ serum were detected, and sacroiliac joint CT grading was performed.To analyze the serum COMP level between three groups, and the correlation between serum COMP and other laboratory indicators was analyzed.ResultsThe serum COMP level in the active group was higher than that in the inactive and the healthy control group(P<0.05).The serum COMP level in the inactive group was higher than that in the healthy patients(P<0.05).Correlation analysis showed that serum COMP level was positively correlated with BASDAI and VAS of spinal pain(P<0.05), but was not significantly correlated with age, course of disease, ESR, CRP, GLB and CT grade(P>0.05).ConclusionsSerum COMP can reflect inflammation and disease activity of AS patients.Combined with serum CRP and ESR results, it can better evaluate inflammation reaction, disease activity and guide treatment of AS patients.
